If you will be visiting us from out of town, please review the following information:

Plan Your Trip to Save Time and Expense 

Many patients like to plan a single visit to southern California where they can have a consultation in our office, have surgery and have necessary follow-up exams within a short time. We can help you do this, as follows:

1. We can try to determine whether you are a candidate for surgery by reviewing your medical records. Please send the form available here to your eye doctor to request that records be sent to us directly.

2. Please send the following items to the address below:
— a copy of your current medical insurance card
— filled out copies of the New Patient Information Form available here
